High Level PROCESS:

Fill out the Formblatt 1 (FB1) as explained below, print and sign, scan signed FB1 and email to Test Director (TD).  A picture from your phone of FB1 will not be accepted.

TD must approve FB1 before you are accepted into the test. FB1 must be correctly filled out using a computer (JGHV requirement)

Test Entry fee is paid by purchasing a "ticket".  TD will provide you a code to input during checkout of purchasing a ticket.

Purchasing a ticket to pay the test entry fee prior to TD accepting you into the test does not guarantee you spot in the test. 

Formblatt 1 (FB1) Process:

1. Click on the link below to download FB1 to your computer. 

2. Open Acrobat Reader

3. THEN open the file you just downloaded. 

• Fill out the form completely, double check registration numbers, birthdates, etc.
• Print the form, sign it at the bottom. Email the completed signed FB1  form with a copy of the dog’s Ahnentafel front and back, (green registration paperwork) to the Test Director.
• Click on Send Now at the top of the form to submit the form to the NA-JGV director of Testing (this is not the same as the Test Director)

4.  Purchase the ticket to pay your entry fee for the test AFTER receiving the code from the TD.

NA-JGV Test Entry Refund policy:
If you withdraw from a test more than 60 days prior to the date the test is scheduled: NA-JGV will refund the test entry fee less the PayPal fee, e.g. VJP $100
If you withdraw from a test less than 60 days before the test is scheduled:  NO REFUND unless the handler or dog is medically unable to run the test.  Must have proof from a veterinarian for the dog and a Medical Doctor’s note for the handler.
